A new white paper from 1000 Lives Plus outlines some of the ways NHS Wales needs to adapt to become a healthcare system that is genuinely patient-centred.
Person Driven Care captures the learning from The Esther Network in Sweden, which is often described as one of the best patient-centred healthcare services in the world.
“The Esther Network has demonstrated that it is possible to devise systems and processes which align to provide patient-centred care,” says Dr Bernadette Fuge, Chair of Age Cymru, and one of the co-authors of the white paper.
